Laugh, Clown, Laugh (1928)

An abandoned girl child is found and adopted by Tito (Chaney), the travelling circus clown. When she grows up and falls in love with a count, Tito realises that he himself fell in love with his step-daughter too.
Tortured by this inner conflict, he goes to see a doctor. There he befriends another patient, who happens to be the count, his romantic rival. Not knowing that they both love the same girl, they honestly try to help each other.
